Registration and application

If you wish to participate in the Duke of Edinburgh Award program you must first register your interest online, then return the following documents to Ms A. Maynard or Ms B. Mifsud at South Sydney High School:

• To register visit the Duke of Edinburgh website
• Download, print and complete the participant application form
• Download, print and complete the participant award plan
• Return completed forms to Ms A Maynard or Ms B Mifsud.

Please Note: If your assessor is not a teacher at South Sydney High School they must fill out the Assessor Commencement Guide. Return this form to Ms A Maynard or Ms B Mifsud.
